Human Models of Selective Insulin Resistance: Alpelisib, Part I
Testing how insulin works in healthy people versus people at risk for type 2 diabetes
📍 Columbia University Irving Medical Center · New York, NY · Get directions →
The goal of this clinical trial is to understand how the blood sugar-lowering hormone insulin works in healthy adults versus those who are at risk for type 2 diabetes. The study will use a drug called alpelisib, which interferes with insulin's actions in the body, to answer the study's main question: does the liver continue to respond to insulin's stimulation of fat production even when it loses the ability to stop making glucose (sugar) in response to insulin.

Fibrosis Lessens After Metabolic Surgery
Testing whether metabolic surgery can reduce liver scarring in obesity
📍 Banner Health Center · Phoenix, AZ · +3 more US sites · Get directions →
Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD), a major global public health concern, is commonly associated with obesity, diabetes, and dyslipidemia. MASLD is currently the most common cause of chronic liver disease affecting about 80% of people with obesity, ranging from simple fat deposits in the liver to Metabolic Dysfunction-Associated Steatohepatitis (MASH), cellular injury, advanced fibrosis, cirrhosis, or hepatocellular carcinoma.

Prevention of Progression of Prediabetes, Obesity and CV Risk
Testing ways to stop prediabetes and obesity from getting worse
📍 University of Texas Health Science Center at San Antonio · San Antonio, TX · Get directions →
The investigators are studying how to help people with prediabetes (Pre-DM) and obesity. The goal is to use new and affordable treatments to bring blood sugar levels back to normal and help participants to lose weight.

Effect of the Inhaled Triple Therapies Over the Small Airway in Biomass Exposure
Comparing inhaled triple versus double medicines for COPD lung airways after wood smoke exposure
📍 Insituto Nacional de Enfermedades Respiratorias "Ismael Cosío Villegas" · Mexico City, Tlalpan · Get directions →
This phase IV randomized controlled clinical trial intend to compare the effect of three close standard inhaled triple therapies and one close standard inhaled double therapy on the small airway in patients with Chronic Obstructive Pulmonary Disease (COPD-B) and chronic bronchitis without obstruction (BCNO) exposed to wood smoke. The treatment phase duration is of 3 months.

📍 Cleveland Clinic Main Campus · Cleveland, OH · Get directions →
Cirrhosis is a form of advanced liver disease that can lead to serious complications, especially when combined with severe obesity. Many patients with cirrhosis also develop a condition called clinically significant portal hypertension (CSPH), which is increased pressure in the veins of the liver.

Diazoxide Suppression Test P&F Study
Testing how insulin helps control blood sugar in people with obesity
📍 Columbia University Irving Medical Center · New York, NY · Get directions →
The goal of this study is to learn about how the hormone insulin controls blood sugar. The main question it aims to answer is about how much insulin the body actually needs to maintain a normal blood sugar level.

Graded Insulin Suppression Test P&F
Testing how insulin controls blood sugar in people with obesity
📍 Columbia University Irving Medical Center · New York, NY · Get directions →
The goal of this study is to learn about how the hormone insulin controls blood sugar in a variety of people. The main question it aims to answer is about how much insulin the body actually needs to maintain a normal blood sugar level.

Individualized Pharmacological Approach to Obesity in Patients With Bipolar Disorder
Testing how to choose weight-loss medicines for people with bipolar disorder
📍 Mayo Clinic · Rochester, MN · Get directions →
The goal of this clinical trial is to identify the specific characteristics (phenotypes) that may be useful to help select the right medication for weight loss, and to study the effect of individualized guided medication in patients with bipolar disorder ages 18-65. The main questions it aims to answer are: * Can the investigators compare the distribution of obesity characteristics (hungry brain, hungry gut, emotional hunger) between bipolar patients and non-bipolar participants (comparing from IRB #24-002375)?

Effect of Insulin Lowering on Lipogenesis
Testing whether lowering insulin for one week changes liver fat in people with obesity and MASLD
📍 Columbia University Irving Medical Center · New York, NY · Get directions →
The goal of this clinical trial is to compare a one-week course of diazoxide (2 mg/kg per dose x 14 doses) and placebo in people with obesity and insulin resistance (IR) with metabolic dysfunction-associated steatotic liver disease (MASLD). The main question it aims to answer are how mitigation of compensatory hyperinsulinemia with diazoxide affects hepatic de novo lipogenesis, a major contributor to MASLD pathophysiology.
