This purpose of this study is to determine the effectiveness of a mobile phone application in helping to control body swelling in patients with kidney problems. The application will help in the day to day adjustments in diuretic medication dosing. Participants in this study will have an application loaded on to their mobile phone by the study team and be taught how to use it over a 2 hour visit. Participants will need to check their blood pressure and weight daily and enter this information into the mobile phone application every day. Participants will need to follow daily instructions in their medication dosing provided by the application. There will be periodic blood testing. This will happen at 2 weeks, 90 days, and up to 4 other times if necessary. At the end of the study there is a 2 hour study visit during which participants will answer a survey. The total length of the study is 90 days.

✅ You may be able to join if…

•Have kidney problems with either nephrotic-range protein in urine (over 3 grams in a day by urine tests) OR stage 4 or 5 chronic kidney disease (estimated filtration rate under 30).
•Have signs of extra fluid in the body, like swollen legs, belly fluid, or fluid around the lungs.
•Can take daily measurements at home (blood pressure and weight) and put them into the app each day.
•Have a working phone number and a smart phone for the study app.
•Can safely follow the plan, including likely not needing dialysis in the next 3 months and having an expected lifespan of 6 months or more.

🚫 You may not be able to join if…

•Weigh less than 100 lbs or more than 300 lbs.
•Have orthostatic hypotension from autonomic insufficiency (a blood-pressure problem when standing).
•Have low potassium at the start (serum potassium under 3.5 mmol/L) or moderate to severe low sodium at the start (serum sodium under 130 mmol/L).
•Have very high creatinine at screening (serum creatinine over 6 mg/dL) or creatinine more than 1.5 times their baseline.
•Cannot measure home blood pressure and weight, or do not have a working phone number/smart phone.
•Expect to start dialysis within 3 months, or have an expected lifespan under 6 months.
•Have a condition that makes it hard to use the app (dementia, illiteracy, or blindness).
•Are pregnant.
•Are in prison.

This is a research study. Every study is reviewed and monitored by an independent ethics board (called an IRB) whose job is to protect participants, and care is overseen by licensed medical staff. You'll be told the known risks before you agree to anything, and you can stop at any time.

Will I get a placebo instead of the real treatment?+

Some studies compare a treatment against a placebo (an inactive version), and some don't. If this one does, the study team will explain your chances of receiving the active treatment before you decide. Nothing is hidden from you.

Do I have to stop taking my current medications?+

It depends on the study. Some let you stay on your current medications and some ask you to adjust them. Never stop a medication on your own — the study team will review everything with you first.

Does it cost anything? Will I be paid?+

Study-related care is provided at no cost to you. Some studies also pay for your time; the coordinator can tell you if this one does. You should never be asked to pay to take part.

Do I need insurance? Will anyone ask about my immigration status?+

No. You do not need health insurance to take part in a research study, and you will not be asked about your immigration status to join. Taking part is about whether you're a medical fit for the study.

What if English isn't my first language?+

You have the right to understand everything before you agree. Study sites can often provide materials or an interpreter in your language — you can ask the coordinator for one.

Is my information private?+

Yes. Your health information is only shared with the study sites you choose to be contacted by, and only to help match and enroll you. It is never sold, and you can ask us to delete it at any time.
